[Upper and Lower Blepharoplasty] As you see on the picture, the patient
complained of uh periocular aging, especially upper and lower eyelids and
wanted rejuvenation with this process so we decided to proceed with an upper
eyelid blepharoplasty skin only and a transconjunctival lower eyelid
blepharoplasty. So this is the video of the procedure, we marked the eyelid crease
based on its natural position and the
[Upper Eyelid Blepharoplasty] area of excess skin is also marked and
contoured to the shape of the face. Lidocaine 2% with epinephrine is
infiltrated and using a colorado tip cautery, we excise the excess skin and I
use this also to contour the upper eyelid, sometimes superiorly we take out
some of the muscle just to contour it nicely to the shape of her face. Now
I typically don’t close that at the same time, I proceed with the lower eyelid
blepharoplasty, lidocaine is injected,
[Lower Eyelid Blepharoplasty] shoehorn retractor and Desmarres is used to
expose the medial and central fat pads, now a Westcott is used and I typically
don’t remove much fat, but basically contour the lower eyelid through this
small transconjunctival incision. Once that is done, we on her, we thought that a
pinch of skin was needed to be removed just to tighten this skin. Now you can
use laser to tighten the anterior lamella with that, but typically I use uh, just a
pinch if the laser is not available. Now at this point, we can go ahead and close
the skin edges and with her, we used a 6-0 nylon suture, which
needs to come out about six days after. On the lower eyelids, the skin edges are
reapproximated with the fast-absorbing 6-0 suture and that’s it. The shield is
removed and here’s a photo of two months post-op
of the patient. [Kami Parsa M.D. Oculoplastic Surgery 465 N. Roxbury Drive, Suite 1001 Beverly Hills, CA 90210 (310)777-8880 www.oculoplastic.info]
